The following table shows the plate layout for IDT for Illumina - TruSeq DNA UD Indexes v2 (96 Indexes, 96 Samples), catalog number 20040870, and the IDT for Illumina - TruSeq RNA UD Indexes v2 (96 Indexes, 96 Samples), catalog number 20040871.
The Index Adapters Pooling Guide, linked here, contains this table along with recommended index combinations for low-plexity library pooling. Some of the indexes are versioned (UDI0015v2, UDI0016v2, UDI0055v2 and UDI0056v2; not indicated in the above table), see the Illumina Adapter Sequences document for more information and for the index sequences for each index pair.
